We have received your form 144/B18, however you were missing sub-sections 14, 15 and 29, and thus we have been unable to process your request. We also note the high number of accident at work reports, ranging from Dr Dahne's "shrunk gerbil" problem to an unusual number of "unplanned vapourisation" incidents. A review is underway.
